Signing this waiver online applies only to the volunteer filling out the form. Any additional volunteers included in the "other people" question above will be required to sign paper waivers on-site at the event.

Your execution of this Volunteer Release and Waiver of Liability (“Release”) is required for the Arizona – Sonora Desert Museum to consider you for a volunteer assignment.

I understand that the Arizona – Sonora Desert Museum encourages and supports volunteers. As a volunteer, I have an important role in providing services and programs to the public. I want the Arizona – Sonora Desert Museum to appoint people like me to serve and to help the Arizona – Sonora Desert Museum. While volunteering, the Arizona – Sonora Desert Museum will provide me with support, supervision, training, and supplies for me to accomplish my assigned tasks.

Therefore, I do freely, voluntarily, and without duress, execute this Release and acknowledge the following terms:

• Waiver and Release. I hereby release, waive, discharge and covenant not to sue the Arizona – Sonora Desert Museum, including its departments, officers, employees and agents, from and in connection with any and all liability that is related to the activities authorized by my work as a volunteer, except as caused solely by the gross negligence or willful misconduct of the Arizona – Sonora Desert Museum. This release, waiver, discharge, and covenant extends to all losses, injury, death, or damage, as well as to any claims or demands related there to, arising from any injury to my person or property, or resulting in my death.
• Indemnity. I hereby covenant and agree to indemnify and hold harmless the Arizona – Sonora Desert Museum, its departments, officers, employees, and agents, from any and all claims and demands, for all loss, injury, death, or damage, that any person or entity may have or make, in any manner, arising out of any occurrence related to the activities authorized in my work as a volunteer, except as caused solely by the gross negligence or willful misconduct of the Arizona – Sonora Desert Museum.
• Medical treatment. I release and discharge the Arizona – Sonora Desert Museum from any claim that arises or may arise due to any first aid, medical treatment, or medical service rendered to me. I understand that I may not be entitled to workers’ compensation.
• Assumption of risk. I understand that my work for the Arizona – Sonora Desert Museum may include activities that may be hazardous. I assume the risk of injury or harm in those activities I choose to do and release the Arizona – Sonora Desert Museum from all liability for injury, illness, death, or property damage occurring from my work for the Arizona – Sonora Desert Museum, except as caused solely by the gross negligence or willful misconduct of the Arizona – Sonora Desert Museum.
• Insurance. The Arizona – Sonora Desert Museum does not have responsibility for providing any health, medical, or disability insurance coverage for me. IT IS MY RESPONSIBILITY AS A VOLUNTEER TO ENSURE I HAVE MEDICAL/HEALTH INSURANCE.
• Photographic release. I grant to Arizona – Sonora Desert Museum the right to use photographic images and video or audio recordings of me that are made by the Arizona – Sonora Desert Museum or others during my work assignment for Arizona – Sonora Desert Museum. The Arizona – Sonora Desert Museum shall have the sole right to receive royalties, proceeds, and other benefits from use of these photographs or recordings.
• Copyright laws. I understand that showing videos in public that are intended for home viewing is prohibited under the U.S. copyright laws.
• Discrimination laws. I agree to follow the Arizona – Sonora Desert Museum’s anti-discrimination policy along with state and federal laws that forbid discrimination in employment, education, housing, public accommodation, law enforcement, or public service based on a person’s religion, race, color, national origin, age, sex, marital status, height, weight, or disability.
• In-kind service. The Arizona – Sonora Desert Museum is eligible for some grants that require us to match the dollars received from the grant. Many of these grants allow the Arizona – Sonora Desert Museum to use in-kind services as a portion of this match instead of actual dollars. I consent to the use of my volunteer time as an in-kind service to help the Arizona – Sonora Desert Museum earn its match for some grants from federal or other sources.
• Other. I agree that this Release is intended to be as broad and inclusive as permitted by the laws of Arizona and that this Release is governed by and will be interpreted according to the laws of Arizona. I consent to the jurisdiction of the Arizona Superior Court in Pima County for purposes of resolving any disputes related to this Release and agree that the Arizona Superior Court in Pima County shall be the sole forum for resolution of any such disputes.
